Recreational Camp Opening Notification Form

Submit this form at least 14 days prior to proposed opening. If you have questions, contact Public Health at 1-800-265-7293 ext. 4753.

If you operate a Recreational Camp please provide the following information and indicate how you will be operating this season:

Do you have a kitchen/cafeteria?
Do you have overnight sleeping quarters?
Do you have a pool?
Do you have waterfront withing the recreational camp?

Please indicate what is applicable to your camp and what you are opening this season.

Have you emailed your Recreation Camp Safety Plan to phi.intake@wdgpublichealth.ca?
Does your plan include Outbreak Management?
Does your plan include hand and respiratory hygiene education?
Does your plan outline how to handle symptomatic or ill staff and campers?
If you have a pool or a waterfront, do you have qualified lifeguard supervision?
Do you have a communication device readily accessible from the waterfront?

Drinking Water System

Have you completed opening seasonal drinking water maintenance?
Have you completed your pre-opening drinking water samples?
Have you submitted the drinking water sample report by email to phi.intake@wdgpublichealth.ca?

The information on this form is collected under the authority of the Health Protection and Promotion Act in accordance with the Municipal Freedom of Information and Protection of Privacy Act and the Personal Health Information Protection Act.
