Sight for Success Vision Program

WDG Public Health is committed to focusing on children’s health. Ocular health is essential to the growth and development and long-term health outcomes of children in our community.

The Sight for Success program promotes childhood ocular health by providing a subsidy for glasses (up to $160) to students in Grade 1-12 in Wellington County, Dufferin County and the City of Guelph with an identified financial need.

To qualify, students must:

• Currently be enrolled full-time at a school in the WDG region;
• Complete a comprehensive eye exam; and
• Meet the financial eligibility

How to receive the Sight for Success subsidy

Step 1: Student completes an OHIP-covered comprehensive eye exam within 6 months.

Step 2: Once the student has an identified need for glasses, the parent/guardian/student can fill out the eligibility form. One form can be completed per family (up to 4 students).

Step 3: A public health nurse will reach out via phone call to discuss eligibility and vision support programs.

Step 4: If the student is deemed eligible, the public health nurse will send confirmation of eligibility to a Sight for Success provider.

Step 5: The student can purchase glasses at the selected provider and receive a subsidy of up to $160. The parent/guardian/student is responsible for any additional cost.
